This resource has been adapted from assessment forms originally developed by staff at the Safer Young Lives Research Centre at the University of Bedfordshire. This resource takes on board findings from research undertaken as part of the Our Voices programme, particularly around the need for young people to be involved in initial discussions around risks, needs and benefits.
